Family Support

This service is designed to provide support services that are not available through other programs. The individuals who receive services through this program have certain problems that require the coordination among various branches of the Departmnent of Human Services.

We are able to assist families in obtaining such things as:

• Mobility Devices
• Communication Assists Training
• Devices to assist with self-care
• Aides to enhance employability

Pathfinder is the entity that oversees the array of services after the interdisciplinary team agrees upon the services to be provided by the various departments. This program may include school-age children. All services and goals must be reflected in an approved Family Service Plan.
