|Request for Information Policy|
When a request for information is made, the Release of Information must be signed by the individual, if competent, or by his/her guardian. If the individual served is a child the release of information must be signed by his/her custodial parent/guardian.
The release form must state what information is requested from the individual's master file. Information pertaining to psychological or medical status will not be released to anyone unless the facility has specific written permission from the individual/guardian to a specified person(s).
The individual, if competent and of age, or guardian has a right to review the contents of the record but the Center has the right to schedule a time for such a review and to have an employee present at the time of such review.
There may be a nominal fee for copying records.
