Trained aides provide medically necessary tasks for assisting with personal hygiene/care activities, bladder and bowel requirements, self-administered medications, meals which may include shopping and preparation, household services essential to helping an individual remain in his/her home, and transportation to access medical and other services. |
A case plan is developed by the recipient and the provider, which is signed by the recipient's physician. This enables Pathfinder to provide almost any service necessary to enable a person to remain at home. |
