This service enables the family to select a trained person to assist them in accessing the services a family feels will be necessary to the well-being of the family members.	This program has been extremely valuable in helping families as a child transitions from preschool to the public schools, and for older children as they leave the public schools.
A plan is developed that must be signed by the recipient's physician before targeted case management is provided.
