Apartment Living

Intermediate Care Facilities
10 facilities
Intermediate Care Facilities (ICF/IID) offer residential care for adults who need more support than a basic room and board setup. These facilities provide health-related services while fostering independence and community involvement in a comfortable, structured environment.

Residential Services Office
The Residential Services Office coordinates housing programs, staff training, and individualized support plans for Pathfinder’s residential facilities. This office ensures consistency in quality care and service delivery across the state, providing seamless experiences for both residents and staff.
